Case Manager Together Home Program

We are seeking a Case Manager â Together Home Program for a 6-month contract role, starting ASAP. Monday to Friday, 35 hours a week or compressed working 9-day fortnight. No on-call or weekend work. $85k base + Super + Salary Sacrifice. You can be based at the Paramatta or Penrith offices. Opportunity to work from home. Fleet cars are provided.

Our client is an award winning Not-for-Profit that specalises in providing community and social housing to individual and families, seeking a place to call home. They have several very successful programs, and their most recent program is, Together Home program.

If you are passionate about working with some of Sydneyâs most vulnerable people, this is the role for you. The Together Home program specifically works with rough sleepers and people in temporary accommodation. As the Case Manager your role is to provide, strength based, client centered, trauma informed wrap around case management. Supporting individuals transitioning from temporary accommodation to private rental accommodation. You will assess and coordinate client referrals, complete interviews, and client intake. Develop and support case plans and referrals in collaboration with clients, including case review and exit planning. You will also link clients with training and employment opportunities and other support services including local health districts, helping them to build good working relationships with the services. The goal of the Together Home program is to help improve clientâs overall wellbeing, independence, and social inclusion.

What you need to be successful:

  • Experience and knowledge of the social and community housing sector

  • Strong case management experience and exposure to working with clients with a variety of complex needs and diverse backgrounds

  • Outstanding communication skills, active and empathetic listening skills and strong written skills

  • Knowledge and understanding of issues that contribute to homelessness and rough sleeping

  • A willingness to travel and work across a number of locations to support your clients

  • ability to work independently and as part of a team

  • ability to mediate under pressure to resolve differences using a strengthâs-based approach.

  • current unrestricted, Australian driversâ licence

  • fully vaccinated (2x minimum) against Covid-19

Checks & Qualifications:

  • Tertiary qualifications in Social Work, Psychology, Welfare, or related areas

  • Australian Driverâs License

  • Evidence of COVID vaccinations and booster vaccinations

  • WWCC or willingness to obtain one

  • NDIS Check or willingness to obtain one

  • National Police check
