Duties will include:

⢠Working with clients with disability and/or mental health conditions, in their homes and community

⢠To assist clients to understand the NDIS funding

⢠Providing advice and support on services available to the client through the NDIS

⢠Support the client to identify their personal goals and desires

⢠Develop a care plan that meets the clientsâ goals

⢠Develop and maintain relationships with all service providers

⢠Provide ongoing reporting on the clientâs progress

⢠Working with service providers to ensure your clients care needs are meet

What you need:

⢠NDIS or Local Area Coordinator experience

⢠Minimum Certificate 4 in Mental Health, Disability Services or 2 years working in paid mental health roles

⢠A current drivers licence

⢠National Police Check

⢠NDIS Worker Orientation Module Certificate

⢠COVID Vaccination certificate or medical exemption
